In 1890, a state commission began plans for the future asylum with property purchased from the estates of Judge Robert Bishop and Moses Bishop, as well as the Morrill farm in the north end of Medfield. Within two years, Massachusetts, through an act of the state legislature, founded the Medfield Asylum.

The campus would ultimately expand to over 900 acres and nearly 60 buildings, but at the time of the initial purchase it had 22 buildings over several hundred acres. That same year, civil engineers began laying the groundwork for the facility. By 1894, construction had begun, and the bare bones of the new asylum took root.
